Longevity Optimization Audit (LOA)
Health Consulting Consent and Acknowledgment

This Consent and Acknowledgment ("Agreement") is entered into by and between the undersigned client ("Client") and Gladden Longevity ("Company") in connection with the Client's voluntary participation in the Longevity Optimization Audit ("LOA") Health Consulting Program.

Purpose of the Program

The Longevity Optimization Audit (LOA) is a non-medical health consulting service intended to provide a broad review of health-related information made available by the Client. The purpose of the program is to offer general wellness insight, education, and health-oriented guidance based on information the Client chooses to provide.

This program is designed to support the Client in better understanding general health patterns, wellness priorities, and potential areas for further discussion with the Client's own licensed treating providers.

Nature of the Services

The Client understands and agrees that the LOA is limited to health consulting only. The services provided through this program may include:

• Review of health history and related records provided by the Client

• General wellness discussion

• Broad health education

• Non-diagnostic health consulting observations

• General recommendations for the Client to consider and discuss with their own treating providers

No Physician-Patient Relationship

The Client understands and agrees that participation in the LOA does not establish a physician-patient relationship, provider-patient relationship, or any other clinical treatment relationship with Gladden Longevity or any of its physicians, providers, contractors, or staff.

• The Client further understands and agrees that:

• This program is not medical care

• This program is not a substitute for care from the Client's own physician or other licensed healthcare providers

• No diagnosis will be made

• No treatment plan will be prescribed

• No medical advice is being given

• No ongoing clinical management is created by participation in this program

No Practice of Medicine

The Client acknowledges that the LOA is limited to health consulting and wellness review only and is not intended to constitute the practice of medicine, nursing, or any other licensed healthcare service.

The Client understands that Gladden Longevity, through this program, is not undertaking to diagnose, prevent, treat, monitor, manage, or cure any disease, condition, or medical complaint.

No Labs, Prescriptions, or Medical Orders

The Client understands and agrees that, through the LOA:

• No laboratory testing will be ordered

• No prescriptions will be written

• No medications will be prescribed

• No imaging, procedures, or medical treatments will be ordered

• No prior authorizations, medical clearances, or disability determinations will be provided

If the Client desires medical evaluation, diagnostic testing, treatment, prescriptions, or clinical follow-up, the Client must seek those services through their own licensed treating provider or through a separate clinical relationship, if applicable and separately established.

Client Responsibility

The Client understands that participation in this program is entirely voluntary and is being undertaken by the Client of the Client's own free will.

The Client is solely responsible for:

• Deciding whether to participate in the program

• Deciding whether to act on any information discussed during the consulting process

• Seeking appropriate medical evaluation and treatment from the Client's own licensed providers

• Obtaining emergency care when needed

The Client agrees to consult their own physician or licensed healthcare provider regarding any medical concerns, symptoms, diagnoses, medications, or treatment decisions.

Not for Emergencies

The Client understands that the LOA is not intended for urgent or emergency medical needs.

If the Client is experiencing a medical emergency, the Client should immediately call 911 or seek care from the nearest emergency room or licensed medical provider.

No Guarantee of Outcome

The Client understands and agrees that Gladden Longevity makes no guarantees, warranties, or representations regarding any specific health outcome, result, improvement, or benefit from participation in the LOA.

Non-Refundable Fee

The Client understands and agrees that all fees paid for the LOA are non-refundable.

By ticking the box, the Client acknowledges and agrees that:

• The Client is voluntarily electing to participate in this program

• The Client understands the nature of the services

• The Client understands that no physician-patient relationship is being established

• The Client understands that no refunds will be issued

Acknowledgment and Consent

By ticking the box, the Client acknowledges that they have read and understand this Consent and Acknowledgment in its entirety, have had the opportunity to ask questions, and voluntarily agree to participate in the Longevity Optimization Audit under the terms stated above.
